Please complete the enclosed Registration Form and send it together with your payment to the Conference Secretariat. Payments can be made by bank draft or bank transfer to account #142-472330, Bank Hapoalim, Branch no. 780, Itzhak Sade’ Street, Tel Aviv or by Credit Card (Visa/Mastercard/Diners/American Express only).

Cancellation Policy
Refund of registration fees will be made if written notification of cancellation is received as follows:
Until July 9, 2003 – Full refund less bank charges and US$100 handling fees.
From July 9, 2003 until July 19, 2003 – 50% refund less bank charges.
After July 19, 2003 – No refund

Accommodation

The Organizing Committee has appointed Ortra Ltd., a registered travel agent, as the official organizer and travel agent for the Conference.

Rooms have been reserved at the Kibbutz Shefayim Hotel at reduced rates. Double and single occupancy is available. Participants are invited to make additional tourist services reservations as desired, such as hotel extensions, regular tours, private car tours, car rentals, touring in limousines with driver/guide, etc.

You are kindly requested to make reservations on the attached “Tourist Services Form” and return it with payment to Ortra Ltd. Payment to Ortra Ltd. can be made by bank draft or bank transfer to account no. #142-472330 at Bank Hapoalim, Branch no. 780, Itzhak Sade’ Street, Tel-Aviv or by credit card (Visa, Mastercard, Diners and American Express only).

Tourist Services Cancellation Policy
In case of cancellation, payments will be refunded in full, except for US$100 cancellation fee plus bank charge, if notification reaches Ortra by July 9, 2002. Afterwards US$100 cancellation fee plus one night hotel charge and bank charges will be retained.
